Parents should call the attendance line at 763-252-6130 before 9:00 a.m. of the day your child will be absent. This phone number is available 24 hours a day to leave a message. Please leave the following information on the recording:

Date your child will be absent
First and last name of your child
Reason for the absence
Name of the parent/guardian who is calling

If you know your child(ren) will be gone more than three consecutive days, you will need to fill out an extended absence form four to six weeks in advance. Please send the Extended Absence Form to Ms. Moffatt.

Need to Know

Chromebook Return + DaVinci Materials Drop Off, Monday, June 1

Many of you are using DaVinci learning materials including Chromebooks, workbooks, reading books, and other items for accommodations. Those things will need to be returned to the school before the summer begins. Monday, June 1 is the day that we will be accepting those supplies back to the school. You will need 2 things to return your Chromebook:

This Chromebook Return Form will need to be completed and returned with the computer.

All other DaVinci learning materials can be dropped at the east door on Monday, June 1. There will be collection bins and carts for you to place items on. Please return these things in a sealed Ziploc bag with your child’s name clearly marked on the bag.

Parent Survey

The annual parent survey will be emailed out next week. Please take the time to give us your feedback so we can do our best to support our students and families.

Summer Suggestions from Nurse Julie:

Vision screenings were not able to be completed at school this year by the doctors from 20/20 Eyecare & Optics of Blaine. If you have concerns or want your child to have an exam, you may contact them at (763)755-9380. Screenings will hopefully be completed for grades 1, 3, 5, and 7 in the fall.
Contact your doctor’s office to see if your student is due for any immunizations. The immunizations required for the school are Tdap, MMR, Hep B, Polio, and Varicella (Chicken Pox). You or your doctor can fax your updated immunization record to DaVinci at (763)767-7817 and I will update your student’s file. Please remember that new medication forms will be required for the 2020-2021 school year for anyone who needs a medication at school. The prescription medication authorization form requires both a doctor’s signature and a parent/guardian signature. The over the counter medication authorization form requires a parent/guardian signature only. The forms and medications can be turned in at the back to school night or when school begins. Medications cannot be given without these signed forms. All health forms can be found here.

Yearbooks

There are a handful of extra yearbooks available for purchase in the office. If you didn’t order one in advance and would still like one, you have 2 options.

1. Come to the front door of the school any time from 8-4 on Monday, June 1. This is Chromebook and materials drop off day and we will have staff available to accept your check for a yearbook (while supplies last), or

Food Service End of the Year Reminders

Our final grab and go meal pick up day for the school year will be on Monday, June 1. So our food orders are accurate, please fill out this form.

Cafeteria negative balances must be paid no later then Tuesday, June 30, 2020. If you need to set up payment arrangements, please email Cassandra Anderson. If you have a positive cafeteria balance the remaining balance will roll over for the following school year. To receive a credit or transfer funds to another account, please fill out the form below.
